Practice Policies
Hello and welcome to FortaMind! We are looking forward to working with you and we want you and all our patients to receive an equal and optimal treatment experience. To ensure that, we have put these practice policies in place that hopefully will clarify most questions that arise. Please read and be familiar with them so you can know what to expect in our clinic.
  • Business hours for FortaMind are Monday through Friday from 9AM-5PM. Individual providers may choose to see patients or respond to communications outside of these hours, but responses from the practice or your provider should usually be expected only during business hours. For emergencies that require immediate care at any time, you should call 911 or go to the nearest emergency room.
  • For ease and clarity, all future appointments should be scheduled online through our practice website (www.fortamind.com) or can be scheduled with your provider during your current session.
  • Please take note as to whether you have scheduled an in-office or virtual (video) appointment.
  • The frequency of appointments depends on individual treatment needs. However, to remain active in this practice, patients are required to schedule and be seen in clinic at least every 12 months. Patients who are prescribed DEA controlled substances must be seen by their provider at least every 6 months to continue treatment with those medications.
  • Treatment discussions, including that of potential new medications or dosage changes, should take place during scheduled clinic appointments. No medication changes will be made via text/email (unless previously planned with your provider). Urgent matters requiring communication or services outside of scheduled appointments (ie. early refills, completion of forms/letters, etc.) may incur additional fee-for-service charges.Prescription refills needed between scheduled appointments should be requested from your pharmacy, who then can electronically request and receive them from your provider. Please contact your pharmacy first to check for available refills or to have them initiate a refill request before contacting your provider.
  • Our practice is only in-network with Blue Cross Blue Shield PPO and Blue Choice PPO plans. If you have an in-network insurance plan, we will submit your claim for medical services. Please contact your insurance provider for questions regarding services covered by your plan and your copay and deductible amounts.
  • You will be responsible for paying any remaining copay or deductible amounts owed, which will be billed to you later.
  • Out of network or self-pay patients will be charged the fee-for-service amount on the day of your appointment. Our current fee schedule is listed on our website and is posted in our clinic.
Cancelation Policiy
Appointments that are cancelled with less than 1 full business day’s notice, or are not attended as scheduled (ie. no-show), will be charged a late cancellation fee (as noted in the fee schedule) directly to the patient that must be paid before another appointment will be scheduled.
Please note: 1 business day’s notice means that Monday appointments must be cancelled by the previous Friday.
